Spondyloarthritides predominantly affect the axial skeleton and rarely lead to peripheral joint involvement.
The entire spine can be affected by the inflammatory changes. Enthesitis (inflammation of the tendon insertions) is common, most often affecting the Achilles tendon.
The most common extraskeletal occurrence is anterior uveitis (iridocyclitis - inflammation of the iris).
Today, a distinction is made between axial and non-axial spondyloarthritis.
Therapeutically, TNF antagonists are used in addition to non-steroidal anti-inflammatory drugs and intensive physiotherapy. In the case of exclusively peripheral joint involvement without axial skeletal involvement, a therapy attempt with sulphasalazine can also be made.
The decision about the therapy is made after the examination.
